Working as an analyst for one of the world’s largest banks is tough. So, Austin was taken aback as his eye freckle threatened to be more interesting than the deals he presented to clients. Or that is how he felt because people would always stare at it instead of him.
Nevi are often nothing more than birthmarks. However, in Austin’s case, the nevus didn’t appear until 2018. It didn’t take off on a growth spurt until several years later. Like many people with eye freckles, Austin began to get tired of the unwanted attention.
“When they started getting bigger, I was afraid to look to the left,” says Austin, a Wells Fargo analyst, now 25. “It was awful at meetings when people would shift their glances at my eyes rather than me.”
What is an eye nevus?
An eye nevus is a common, usually benign (non-cancerous) pigmented growth in the eye, similar to a mole on the skin. It is caused by an accumulation of melanocytes, the cells responsible for producing pigment. Eye nevi can appear in different parts of the eye, including:
- Iris nevus: Found on the colored part of the eye.
- Choroidal nevi: Located beneath the retina in the choroid, a layer of blood vessels in the back of the eye. Most are benign, but they can rarely be choroidal melanoma.
- Conjunctival nevus: Found on the surface of the eye, typically on the white part (sclera).
Should you be concerned about a birthmark on your eye?
Most eye nevi are harmless and do not cause symptoms. However, they should be monitored regularly by an eye doctor because, in rare cases, they can develop into melanoma (a type of eye cancer). Signs that may indicate concern include changes in size, shape, color, or fluid leakage.
If you have an eye nevus, regular eye exams with imaging (such as optical coherence tomography or ultrasound) can help track any changes over time.

What to do with a brown spot in your eye?
Removing a cosmetic nevus is straightforward, but experience makes all the difference in the patient’s outcome. Dr. Assil employs a gentle thermal brushing technique to gently extract the pigment from the surface of the conjunctiva, a thin, transparent membrane that protects the eye.
Austin quickly booked an appointment and met with Dr. Kerry Assil to discuss the removal of his eye freckle. Based on his examination, Dr. Assil diagnosed a choroidal nevus and recommended using the brush technique due to Austin’s dry eye syndrome. Other treatment options include laser treatments and incisions. Nevus surgery typically has no side effects.
“Using the brush technique is far less invasive than making an incision in the eye,” explains Dr. Assil. “It’s also a faster healing process.”
The nevus removal
The surgery was uneventful. Before the procedure, Austin was given a mild sedative to calm any nerves he might have and some numbing drops for his eyes. After less than ten minutes, Austin felt like a new man, with his pesky eye freckles a thing of the past.
Recovery is typically quick and painless, with no trace of the prior nevus. Because this treatment is considered cosmetic eye surgery, insurance does not normally cover it.
Now, Austin and his clients can focus on the world of high finance. “I’m super satisfied with the results,” says Austin. “I noticed a difference right away.”
