Dermal Fillers Done Right: Ophthalmologist-Led Precision

As we age, we gradually lose deeper subcutaneous fat. This is the layer that gives our face a more youthful appearance. Dermal fillers are injectable treatments that help restore volume, smooth out wrinkles, and enhance our facial contours. They are a minimally invasive option for facial rejuvenation, offering noticeable results with zero downtime.

“Short of having a facelift, dermal fillers are one of the most effective skincare options to reduce the signs of aging,” says Dr. Gaur. “They restore the natural contours and fullness that time takes away.”

How do dermal fillers work?

Dermal fillers are available under well-known brand names such as Juvederm, Restylane, Radiesse, RHA, Defyne, Lyft, Perlane, and Voluma.

Many of these products contain hyaluronic acid, a naturally occurring substance in the body that helps retain moisture and stimulate collagen production. This improves skin firmness and elasticity over time.

What can dermal fillers treat?

Fillers may be used for:

  • Softening wrinkles, fine lines, and facial folds (like nasolabial folds and marionette lines).
  • Correcting volume loss by adding volume to the cheeks and restoring mid-face fullness.
  • Plumping thin or aging lips.
  • Filling in shallow areas of the face or hollow under-eyes (tear troughs).
  • Improving the appearance of recessed scars.
  • Enhancing jawline definition and contour.

The team at AG Aesthetics (a division of the renowned Assil Gaur Eye Institute)  carefully customizes each treatment to your facial structure and aesthetic goals.

When will I see results?

Results from dermal fillers are visible immediately after your treatment. You’ll notice enhanced volume and smoother contours right away. Some initial swelling is normal and will subside within a few days, revealing your final results. Depending on the type of filler used and the area treated, results typically last between 6 to 18 months.

Are dermal filler injections safe?

Dermal fillers are widely used and generally safe when administered by experienced and medically trained providers like Dr. Gaur and Dr. Adele. However, as with any medical procedure, it’s important to understand both the common and rare risks involved.

Most patients experience only mild side effects that resolve quickly on their own, such as:

Common side effects

  • Redness or slight bruising at the injection site.
  • Temporary swelling or tenderness.
  • Mild discomfort or firmness in the treated area.

These effects typically subside within a few days and are considered a normal part of the healing process.

Rare complications

While serious complications are extremely rare, it’s important to understand the risks. Having said that, you can feel confident that our team of injectors is highly trained and fully equipped to promptly and effectively recognize and manage these enhancements.

“Most people are unaware that one of the rare but serious complications of facial filler injections, particularly near the eyes and forehead, is vision loss,” explains Dr. Gaur. “This can happen because the face, especially around the eyes and forehead, contains many small arteries that supply blood to critical areas, including the eyes.”

If filler is accidentally injected into or near one of these blood vessels, it can block blood flow to the retina. In extremely rare cases, this can lead to blindness. Even with the most skilled provider, this can occur, but you must always confirm that the provider has an emergency protocol in place.

“Every BOTOX provider must have an ophthalmologist on call to deal with emergencies, and many providers simply do not. In our practice, we have a comprehensive emergency protocol for these situations, including available filler reversal product, urgent eye evaluation and treatment, and of course, we are ophthalmologists, so we are immediately available,” explains Dr. Gaur. So, there are no delays, referrals, or transfers. This level of preparedness and expertise is critical for your safety.”

The ophthalmological advantage in aesthetics: precision, safety, and great expertise

When it comes to dermal filler treatments, especially in the delicate areas around the eyes, tear troughs, and mid-face, the training and expertise of your provider can make all the difference. At AG Aesthetics, the team at Assil Gaur Eye Institute brings the precision and safety standards of an ophthalmologist to every filler treatment.

You’re not visiting a typical med spa. You’re receiving care in a physician-led medical practice where treatments are guided by surgical-level precision and deep anatomical expertise. Every procedure is performed with the safety standards, clinical judgment, and emergency readiness of a board-certified ophthalmologist.

Here’s why having a board-certified ophthalmologist perform your dermal filler treatments provides an unparalleled advantage:

Millimeter-level precision

Ophthalmologists work in millimeters daily, performing intricate eye surgeries and making precise measurements of ocular structures. This level of precision is directly applied to filler injections, ensuring each placement is exact, balanced, and tailored to your unique facial anatomy.

Deep knowledge of periorbital vascular anatomy

The area around the eyes contains a complex network of delicate blood vessels. Dr. Gaur’s extensive training in the anatomy of the eye and surrounding structures means she knows exactly where these vessels are located and how to avoid them. This is especially critical when treating the tear troughs, under-eye hollows, and temple areas where precision matters most.

Understanding of and experience with delicate tissue layers

The tissues around the eyes are among the thinnest and most sensitive in the entire face. Ophthalmologists spend years studying and working with these delicate structures.

This experience translates to a gentle, refined injection technique that minimizes discomfort, bruising, and swelling while maximizing safety and aesthetic results. For example, our team can inject fillers to the ideal depth and volume, avoiding complications such as lumpiness, discoloration, or the dreaded Tyndall effect (a bluish tint under the skin).

Emergency preparedness for vision-threatening complications

As we touched on earlier, while extremely rare, one of the most serious complications of facial filler injections, particularly near the eyes and forehead, is vascular occlusion, which can lead to vision loss. This is why having an ophthalmologist perform your treatment is so important.  Our team is trained to recognize, respond to, and manage any eye-related complications immediately, without delays or referrals.

Comprehensive Understanding of Facial Proportions and Symmetry: Ophthalmologists are trained to evaluate facial symmetry and balance as part of diagnosing and treating conditions like ptosis (drooping eyelids) and facial nerve disorders. This trained eye for proportion ensures that your filler treatments enhance your natural beauty while maintaining harmony across your entire face.

“My training as an ophthalmologist gives me a unique perspective when performing filler treatments,” says Dr. Gaur. “I don’t just see the surface. I understand the intricate layers, the vascular pathways, the muscles, and the delicate tissues beneath. This allows me to deliver results that are beautiful, natural, and, most importantly, safe.”

What can I expect during my treatment?

Before Your Treatment

Our team will conduct a thorough consultation to understand your aesthetic goals, evaluate your facial structure, and provide a personalized treatment plan. We’ll explain which areas can benefit most from fillers and discuss realistic expectations. Avoid blood-thinning medications like aspirin or ibuprofen for a few days before your appointment to minimize bruising.

During Your Treatment

The treatment typically takes 15-30 minutes. A topical numbing cream is applied to the treatment area to minimize discomfort. Many fillers also contain lidocaine, a local anesthetic, for added comfort during the injection. You may feel slight pressure or a brief pinching sensation, but most patients find the procedure very tolerable.

After Your Treatment

You can return to normal activities immediately. However, you may have some mild swelling, redness, or bruising. To optimize your results:

  • Avoid touching or massaging the treated area for 24 hours.
  • Avoid strenuous exercise for 24 hours.
  • Avoid excessive heat (saunas, hot tubs, intense sun exposure) for 48 hours.
  • Avoid alcohol for 24 hours to minimize swelling.
  • Apply ice packs gently if needed to reduce swelling.

Dermal Fillers FAQs

How long do dermal fillers last?

The longevity of dermal fillers depends on the type of filler used and the area treated. Most fillers last between 6 and 18 months. Fillers in high-movement areas, such as lips, tend to break down more quickly. Those in less mobile areas, like the cheeks, last longer. Our team will discuss the expected duration during your consultation

Do dermal fillers hurt?

Most patients experience minimal discomfort. Lip augmentation may be mildly uncomfortable, but most find it very tolerable. A topical numbing cream is applied before the procedure, and many fillers contain lidocaine for extra comfort. You might feel slight pressure or a brief pinch, but the process is generally well-tolerated

Can fillers be reversed?

Yes! Hyaluronic acid-based fillers, such as Juvederm and Restylane, can be dissolved with an enzyme called hyaluronidase. This enables adjustments or complete reversal if necessary. Our team keeps this reversal agent available as part of their emergency protocol

Will fillers make my face look overfilled or unnatural?

Not when performed by a skilled provider like Dr. Gaur or Dr. Adele. The goal of dermal fillers is to restore natural facial volume and subtly enhance your features. Our team takes a conservative, artistic approach to ensure your results look refreshed and natural; never overdone.

Can I combine fillers with BOTOX?

Absolutely! Many patients combine BOTOX and fillers for comprehensive facial rejuvenation. BOTOX relaxes dynamic wrinkles caused by muscle movement, while fillers restore lost volume and smooth static wrinkles. Together, they create a balanced, youthful appearance.

Are fillers safe during pregnancy or breastfeeding?

As a precaution, dermal fillers are not recommended during pregnancy or breastfeeding, as there is limited research on their safety during these periods. It’s best to wait until after pregnancy and breastfeeding are complete.
