What causes eyelids to droop?

Perhaps you, or someone you know, has visible drooping of one or both upper eyelids that causes the eye to appear partially closed because the eye’s levator muscle can’t open wide enough to fully expose the iris (the colored central part of your eye). This condition is called Ptosis (TOE-sis).

Eyelid ptosis is caused by an anatomical defect that you’re born with it or an acquired functional problem caused by nerve damage, age, or certain medical conditions (stroke, Horner’s syndrome, myasthenia gravis, diabetes, a tumor, or stye). Below are before and after pictures of cases performed by Dr. Adeleh Yarmohammadi (or “Dr. Adele” as she is known by her patients).

Brow Ptosis and Quadruple Blepharoplasty

This patient underwent upper and lower blepharoplasty with Dr. Adele, an oculoplastic surgeon specializing in natural eyelid rejuvenation. The goal was a subtle transformation that restored a more rested, youthful appearance while preserving the patient’s natural eye shape. The upper blepharoplasty removed excess skin that was causing heaviness of the upper eyelids, resulting in a brighter, more open look. For the lower eyelids, Dr. Adele performed fat repositioning to eliminate under-eye bags and smooth the transition between the eyelid and cheek. A conservative skin-pinch technique refined the lower eyelid skin without altering the natural contour of the eyes. The result is a refreshed, natural appearance with no visible eye bags and no change in eye shape—a hallmark of modern oculoplastic surgery.

Droopy eyelids, known as ptosis, can stem from various causes. In the case of this lovely patient, aponeurotic ptosis was the culprit, where the muscle responsible for lifting the eyelid slipped from its attachment due to long-term contact lens use. This condition often leads to a tired or sleepy appearance. To correct the eyelid droop, they underwent scarless ptosis correction alongside bilateral upper lid blepharoplasty. Here are images capturing the before and one-month post-surgery transformations.
